Nisoldipine is a second-generation calcium channel blocker with dihydropyridine structure similar to nifedipine and high vascular smooth muscle selectivity. Increased vascular selectivity is associated with increased lipophilicity, which is more likely to enter the vascular wall and block voltage-dependent calcium channels. Similar to other calcium channel blockers, nisoldipine inhibits calcium influx by blocking the voltage-dependent calcium channel of the cell membrane, resulting in vascular smooth muscle relaxation, decreased muscle contractility, and negative time-varying effects. Studies have shown that nisoldipine dilates peripheral blood vessels at least 10 times as much as nifedipine. In addition, coronary artery dilation is more effective and selective. The affinity of nisoldipine for inactive calcium channels is higher than that of nifedipine, the separation speed is slower, and the distribution coefficient is larger, so the action time is longer than that of nifedipine.
